Program Year 2026 – 2027 · Fill & Sign Online
This Participation Agreement, Release of Liability, Assumption of Risk, and Indemnification ("Agreement") is entered into by the undersigned parent(s) or legal guardian(s) on behalf of themselves and their participating child(ren) in connection with participation in the St. John Enrichment Co-op ("SJEC" or "the Co-op"). Please read every section, then complete and sign below.
1Voluntary Participation
I understand that participation in SJEC is entirely voluntary. I freely choose to allow myself and my child(ren) to participate in all classes, educational activities, enrichment programs, field trips, social gatherings, service projects, meetings, and other events sponsored, organized, or facilitated by SJEC.
2Assumption of Risk
I acknowledge that participation in homeschool co-op activities involves inherent risks that cannot be completely eliminated. These risks include, but are not limited to:
Falls, slips, trips, and other accidental injuries;
Injuries occurring during indoor or outdoor activities;
Exposure to weather conditions, insects, plants, or animals;
Exposure to communicable illnesses;
Injuries occurring during educational demonstrations, crafts, sports, or recreational activities;
Risks associated with field trips, parks, churches, private homes, businesses, or other third-party venues;
Transportation-related risks.
I knowingly and voluntarily assume all risks, both known and unknown, associated with participation.
3Release and Waiver of Liability
To the fullest extent permitted by Tennessee law, I release, waive, discharge, and covenant not to sue St. John Enrichment Co-op (SJEC), its directors, coordinators, board members, volunteers, instructors, officers, committee members, participating families, churches, facilities, property owners, and agents from any and all claims, liabilities, demands, actions, causes of action, damages, costs, expenses, attorney's fees, or losses arising out of or relating to participation in SJEC activities. This release applies to claims involving personal injury, illness, property damage, emotional distress, or wrongful death arising from participation in SJEC activities, except where such claims result from gross negligence, reckless conduct, or intentional misconduct to the extent such claims cannot legally be waived under Tennessee law.
4Responsibility for Children
I understand that SJEC is a volunteer-led homeschool cooperative and not a licensed childcare provider or school. I remain ultimately responsible for the conduct, supervision, discipline, and well-being of my child(ren), except during periods in which another supervising adult has expressly agreed to supervise a scheduled activity. I agree to promptly pick up my child(ren) at the conclusion of all scheduled activities.
5Medical Authorization
In the event of an emergency, I authorize SJEC volunteers or representatives to obtain emergency medical treatment for my child(ren) if I cannot be reached. I understand that SJEC assumes no responsibility for medical expenses and that all medical costs remain my sole responsibility.
6Medical Information
I agree to provide accurate information regarding allergies, medications, medical conditions, dietary restrictions, and emergency contacts and will promptly notify SJEC of any changes.
7Transportation
Transportation to and from SJEC events is the sole responsibility of each participating family unless otherwise arranged. If I choose to transport or allow my child to ride with another parent or volunteer, I voluntarily assume all associated risks.
8Property Damage
SJEC is not responsible for lost, stolen, or damaged personal property. I agree to be financially responsible for any property damage caused by myself or my child(ren).
10Code of Conduct
I agree that my family will abide by all SJEC policies, safety rules, behavioral expectations, and decisions made by the leadership team. I understand that SJEC reserves the right to suspend or terminate participation if conduct is determined to be unsafe, disruptive, abusive, or inconsistent with the mission of the Co-op.
11Indemnification
To the fullest extent permitted by Tennessee law, I agree to indemnify, defend, and hold harmless SJEC and its directors, coordinators, officers, volunteers, instructors, participating families, host churches, facilities, and agents from any claims, liabilities, damages, judgments, attorney's fees, or expenses arising from:
My own acts or omissions;
The acts or omissions of my child(ren);
My family's violation of SJEC policies; or
Any claim brought by or on behalf of my child(ren), except to the extent prohibited by applicable law.
